SCHOOL NURSE NOTES
Medications
The school has two different medication forms.
Please remember, UNDER NO CIRCUMSTANCES MAY A STUDENT CARRY MEDICINE, EITHER TO SCHOOL OR HOME unless they have a self-medication order on file. This is a state law.
Consider obtaining an order for over the counter medications if your child may be in need for the school year - due to dental procedures, headaches, etc. If there is a form on file, your child can be medicated.
Absences/Late Arrivals
In accordance with the No Child Left Behind Act, and school policy, we need a parent or guardian to call the school if a child will be absent or late. Siblings, for obvious reasons, cannot explain absences. Please follow up the phone call with a letter/note sent in when the child returns to school. If you know in advance that your child will be absent you may send the note in early to the nurses' office. A copy can be made and given to the child's teacher if requested. Documentation helps if absences become an issue.
Illness/Fever
If your child is obviously ill or with a fever, please keep them home until they are well enough to attend school. The school's policy is the child must be fever free for 24 hours prior to returning to school. Other illness that require exclusion are:
Strep Throat requires 48 hours after antibiotics have been started, and fever free for 24 hours before they may return to school.
Diarrhea -Exclusion until episode has passed.

Gvm Notes- Students may be excluded from physical education classes up to 3 days by written parental request. A doctor's note is needed for longer periods of time. Any student excluded from physical education is also excluded from recess and after school sports.
